Re: Permission for publication



Only two of the several institutions at which I used original resources for my recent book had a form for requesting permission to publish from their manuscripts.  One was the Historic Documents Department of the San Francisco Maritime National Historical Park.  The other was the Beinecke  Library at Yale University.  You might try obtaining copies of their forms.  For all the others I used a letter in which I followed the recommendations of the publisher.
