I think you probably are trying too hard to bend the Greek. I don't think we should assume that the different synoptic versions should be harmonized. I think that ἀπολελυμένην probably should be best understood as a passive: "a woman who has been divorced from (her) husband. The question may be...

The citations from Plutarch are relevant, but it should be remembered that Plutarch was a card-carrrying Atticist, one whom you'd expect to be using a dative of agent with a perfect passive.
