Verbs and nouns (anything that can be two columns singular and plural), get the top down treatment. Adjectives and pronouns which have three genders (and hence three columns), get the left to right according to gender and move down according to case. That way when you memorize them you keep the gend...

Well, just a thought. What if one of the answers you have precluded is the answer? But on the syntax question, if the vocative marker fills the same "space" as the article, then it also may be used attributively or not. I would read the use in Sophocles as a simple attributive use.
