Heb 9:15 is the final one.
Unless there's a variant in a manuscript at Heb 12:24 for διαθήκης νέας.
Plus there really is only the one διαθήκην καινήν in the LXX. There are quite a lot of διαθήκη/ν/ς κυριου though (62) and διαθήκη/ν/ς μου (46).

BlueLetterBible has a nifty search function that allows you to combine Strong's numbers in a search for multiple words occuring in the same verse. Probably less time consuming than using Logos or whatever one uses for more intensive searches.

For LXX, I searched for the Hebrew equivalent, with the expectation that the Greek mirrored the Hebrew:

It seems that the other two verses I was counting on being included - Matthew 26:28 and Mark 14:24 - use the phrase in TR/Byz but not in SBL/NAUSB, so there are 2 correct answers dependent on the text one uses.
Blue letter Bible must use the TR or Byz as the basis of the search.
