Title: Moral decision-making in a selected population of rural home health visitors

Abstract: The purpose of this study was to examine the relationship between the subjects' levels of training/home visiting experience, moral judgment development, the number of solutions suggested for a moral dilemma, and the ranking of those solutions. Data were gathered by the use of the Defining Issues Test (DIT), a Practical Solutions Questionnaire, and a Ranking Procedure.

The subjects for the study were 77 females residing in rural counties in West Alabama. The women's names were obtained from the Rural Alabama Pregnancy and Infant Health Program and the Alabama Public Health Department. There were four study groups comprised of (a) trained home visitors with 0 to 6 months of experience, (b) trained home visitors with over six months of experience, (c) professionally trained nurses, and, (d) a control group of women with no training or experience.

The data were analyzed using one-way analysis of variance and Pearson's product moment correlation coefficient. A statistically significant difference was found between the home visiting training/experience levels of the groups using moral judgment scores, the number of suggested solutions to a moral dilemma, and the rankings of these solutions as the dependent variables in separate analyses. No correlation was found between moral judgment scores and the number of suggested solutions to a moral dilemma.

These findings suggest that the professional nursing group tended to have the higher DIT scores. The non-professional and professional home visiting groups listed more solutions to a moral dilemma than did controls. The ranking of these solutions varied among the groups. Implications of this research point to the advisability of further research to explore the possibilities of exposing non-professionals home visitors to moral education training and to including culturally pertinent psychosocial issues in the training of nurses.
