Title: Youth cardiovascular disease risk and prevention: The Minnesota Heart Health Program and the Class of 1989 Study

Abstract: This research project concerns the development of health behaviors related to cardiovascular disease (CVD) during adolescence in a cohort of adolescents from sixth through twelfth grade. The Class of 1989 Study is part of the Minnesota Heart Health Program, a research and demonstration project designed to reduce cardiovascular disease in three communities. This project describes the longitudinal outcomes from one intervention and matched reference community. The specific hypotheses for this project were: (a) the course of CVD risk onset can be altered by a community- and school-based CVD prevention program; (b) CVD health behaviors track from 6th through 12th grades; (c) CVD health behaviors are correlated; (d) psychosocial variables measured at one grade predict CVD health behaviors. Each of the four hypotheses were supported by the results. Smoking rates throughout the follow-up period were significantly lower in the intervention community. Food choices and salt scores were higher throughout most of the follow-up period. Physical activity rates were higher for females, and there was a nonsignificant positive trend for males. Tracking of CVD behaviors was apparent. For physical activity and food choice variables, 6th graders measuring high remained high, and those low remained low. For smoking, students who experimented with smoking were more likely to become and remain a regular smoker than were never smokers. For covariance, students who were exercising infrequently or making poor food choices were also smoking at a higher weekly prevalence. Physical activity and food choices were also related. For prediction of CVD behaviors, the most consistent predictor of future behavior was current behavior. Best friend smoking status and intentions to smoke in the future were also predictive of the onset of smoking. Few other psychosocial variables were predictive of future CVD behavior. The Class of 1989 Study provides optimism for the benefits to youth of long-term, school-based, community-wide programs. Several of these results may be useful for future planning of school and community health education, particularly for the justification of preadolescent interventions to promote healthy behaviors.
