Around $100/year for startup, using your estimate of your gross income
from your farming operation in any given year. The insurance premium rates
are directly based on annual gross income. You make adjustments with your
agent. 

I assume each farm will need its own policy; after all why should you pay
for insurance on products grown by someone else? But I would imagine the
insuror could do a group policy for a CSA run by a number of farms; this
seems to me to be a CO-OP CSA, so they would formally be insuring a farm
cooperative.
