Request Forms



Greetings,

The Archives at the University of Maine at Farmington
is currently revising the request forms that patrons
fill out to request material from the university's
archives. The form currently has patrons write down
their name, address, phone number, status (faculty,
staff, student, or courtesy), purpose of request
(dissertation, article, research paper, book, etc.),
and the information that they are looking for (either
the call number of the material or a description of
the items they are requesting). Is there anything that
should be added or deleted from the request form? What
do other university/college archives include on their
forms? Please feel free to send sample request forms.

Also, do you call the patron when the material has
been retrieved? How many days do you hold the material
for before it is returned to the archives?

Any input will be greatly appreciated!
